Note: If you have any of the accepted insurances plans listed, please send an email to info@empoweredmindsbehavioral.com with the patient’s name, DOB, a copy of your insurance card, and a copy of your driver’s license. You will receive information for the patient portal. Do not make payments online if you have the insurance listed.
