Generally, the first appointment lasts 45-60 minutes.

Please complete the new patient paperwork sent by your provider. A list of all your current and past medications should be brought with you. Please have your medication bottles, a medication list from your pharmacy, or medical records from past physicians. We will need a copy of your ID and insurance card and an acceptable form of payment (Mastercard, Visa, AMEX, Discover, or cash).

All payment is due at the time of service. We accept Mastercard, Visa, AMEX, and cash. Personal checks are NOT accepted.

In the event of a medical emergency, please dial 911 or go to the nearest emergency room.

For psychiatric assessment, patients may be referred to Ridgeview Institute: 770-434-4567.

Weekly/Monthly until the provider or therapist feels the patient is stable under the current treatment plan.

Patients with two or more missed/canceled appointments may be considered non-compliant and possibly terminated from the practice.

Patient cost responsibility is due at the time of service. Payment is due for new patient appointments at the time of scheduling to guarantee the appointment.

Insurance, address, phone number, and pharmacy changes need to be updated with our office.

Completion of Disability paperwork is at the individual provider’s discretion. Referrals for intensive treatment programs may be provided.

Any medication refill requests will be considered on a case-to-case basis, and if the provider agrees, a fee will be applied if refilled without an appointment.

All providers are Independent contractors all fee schedules are the Individual provider rates.
